I have voluntarily agreed to participate in the personal defense and combatives training program that is being presented by, Blauer Tactical Systems, Inc. (hereafter the "Training Program").

I understand that the Training Program will require participants to participate in physical activity related to martial arts, self-defense, and combatives. Participants will actively engage in physical training exercises that may include, but are not limited to, close-quarter combative training drills, ground fighting, joint manipulations, application of pressure points, achieving physical control over others, weapon protection, throws, falls, and scenario simulations. The training may require participants to use training equipment and actively engage in close-quarter street defense or combat exercises. I additionally understand that I may experience emotional or mental stress as a result of my participation in the training. Notwithstanding my awareness that certain risks and dangers exist and May occur during this training, I still wish to participate.

In consideration for the right to participate in this training, I UNDERSTAND AND DO HEREBY AGREE TO ASSUME ALL OF THE ABOVE RISKS AND OTHER RELATED RISKS WHICH MAY BE ENCOUNTERED DURING THE TRAINING, INCLUDING ACTIVITIES IN PREPARATION OF AND/OR SUBSEQUENT AND RELATED TO THE TRAINING.

Accordingly, I on behalf of my heirs, assignees, and/or successors, do hereby agree to hold Blauer Tactical Systems Inc., its directors, agents, and employees harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my person or property, even injury resulting in death, which I now have or which may arise in the future in connection with my participation in the Training Program and in my use of the training equipment provided.

I expressly agree that this Release, Waiver of Liability, and Indemnification Agreement is intended to be as broad as permitted by the applicable laws and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full force and effect. The present contains the entire agreement between the parties hereto and the terms of this Agreement are contractual and not a mere recital.

I acknowledge that my participation in the Training Program does not qualify me to obtain a "certification." I acknowledge that in order to obtain a "certification," I must attend and successfully complete a Blauer Tactical Systems instructor-level course, as well as receive my "post-session" evaluation by Blauer Tactical Systems' Training Team. Once certified, I will then be allowed to use the Training Program's nomenclature, concepts, and training formulae.

As stated in class and on outlines provided to me, I acknowledge that I am strictly prohibited from using Blauer Tactical Systems research to promote myself to or to secure contracts within the law enforcement, military or security communities unless by special arrangement with Mr. Blauer or Blauer Tactical System Inc. Only those certified, by Blauer Tactical System Inc., can teach our methods and associate our name to their business. We reserve the right to inspect any written documentation using our trade names, research, or concepts so that we may verify whether we approve of the manner in which they are being used.

I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND I SIGN THIS RELEASE AS OF MY OWN FREE ACT. This is a legally binding agreement, which I have read and understood.

I additionally certify that I have no condition, whether mental and/or physical, that would prevent me from participating in this activity.
